I tested your example and the uploaded file no name.
Example the file example.doc was saved with the name .doc

Its your code, i only copy the files in to my server and run it without change.

you could test at http://radionova.entucuman.com.ar/upload/index.php

if you upload any .docx document the program will return File Already Exists because all file are saved without name, only the extension
